  1. Intervention costs per capita (total population) reflect reference implementation presented in Table 1 above and Additional file 2. Estimates in the first-row show costs incurred in the first year (i.e. the year the intervention is first introduced), assuming the intervention is only to be deployed for 1 year. The second row gives the average annual economic cost assuming each intervention is implemented annually for 5 years
