  1. aTwo E-2020 countries (Algeria and Paraguay) finalized classification of several cases after selection for the initiative was complete and determined that they had actually achieved their first year of zero indigenous malaria cases in 2014 and 2012, respectively; Paraguay was, therefore, classified as malaria-free in 2015 while Algeria was still malaria-endemic as it had yet to complete 3 years malaria-free
  2. bParaguay is not included in the calculation of the median because it was no longer classified as malaria-endemic in 2016
